We can’t wait to welcome you and your family to Plantation Pointe Dentistry, but before you come to see us for the first time, we recommend taking a moment to read through the helpful information below.

Your First Visit

After being greeted by our friendly front desk staff, you’ll be escorted to a treatment room where an assistant will begin your appointment by taking a few scans and X-rays of your mouth. You’ll then have your teeth cleaned by a hygienist, and afterward, Dr. Wade will perform a complete oral exam. He’ll go over his findings with you, discuss any additional treatments you might need, and answer any questions you may have.

Patient Forms

We want to help you maximize your visit with us, so before you come for your appointment, we recommend that you fill out our patient forms ahead of time. You can download them by clicking on the link below. Just bring the completed forms to our dental office, and we’ll get you in to see Dr. Wade right away! Please call our dental office if you have any issues downloading the forms.

Regular Appointments

Regular checkup appointments typically last about one hour and in order to honor the requests and needs of all patients, we ask that you provide at least 48 hours’ notice if you need to cancel. If you cancel within 48 hours of your appointment time, we will charge a $50 cancellation fee. We will do our best to reschedule your appointment in a timely manner.

Dental Insurance

Yes! If you have dental insurance, then our team is ready to help you maximize your benefits at every single appointment. We are an in-network provider with Delta Dental, and we’re happy to accept all dental insurance plans. Our team will even handle your claims on your behalf so we can make sure that you’re saving as much as possible on your dental care. Payment & Dental Office Policies

You will be able to discuss fees and payment options before treatment begins and we have payment plans to suit different budgets. In an effort to keep fees down while maintaining the highest level of professional care, we have established this financial policy:

  • If full payment is made at the onset of treatment, we will offer a 5% discount since no further bookkeeping fee is needed.
  • To fit your individual needs for extensive treatments, financial arrangements can be made to extend your payments over a period of months through outside funding sources.
  • For your convenience, we accept payment by VISA and MASTERCARD.
